New county project form
Starting for the 2023-2024 year, project leaders have to fill out a form for the county 4-H office and the survey below for East Hills. Since the county office wants the form filled out directly, you can download the new Annual Project Plan (Word doc format) here or on our Google Drive (at East Hills Club Google Drive – if you don’t have access, email Kris). I used their sample document to put in sample/default answers to make it as easy as possible, it is a lot. Email the list or Kris for help!

Once you have completed the form (save a copy to your computer), go to the county 4-H upload form to upload your document (it is a Google form).
